Following the regulations of the Alberta Government for a handicapped parking permit, Monarch Physiotherapy Clinics have allocated a space for handicapped parking. We also issue an exclusive Alberta parking permit for people with disabilities. When you apply for the disability parking placard for the disabled parking lot you should meet the following criteria:
Types of Parking Placards
We can issue 3 parking placards to handicapped people:
- A temporary parking placard issued for anywhere between 3 and 12 months
- A long-term parking placard issued to individuals for a period of 5 years
- A permanent parking placard which can be renewed after every 5 years without the need to provide an additional medical certificate.
You can get just 1 placard for a registered vehicle in your name. We do not issue placards to volunteer vehicle drivers.

Completion of the Application Form to Apply for the Handicap Parking Placard
You can download the application form to apply for the handicapped parking placard by clicking here or obtaining it from our office. It needs to be completed by your physiotherapist based on the evaluation of your specific needs for the parking placard. When the form is complete, you can take it to the parking registry office where an agent will issue you the handicapped parking placard.
Stolen, Lost, or Expired Parking Placard
In case the parking placard is lost or stolen, you need to report it to the parking registry office and they will issue a replacement form. Fill in the form and submit it to apply for the replacement of the handicapped parking placard.
The expired parking placard can also be renewed from the parking registry office.
Canceling Your Parking Placard
If you no longer need your handicap parking placard, you need to obtain a parking cancellation declaration form from our office. Fill in the form and submit it to the parking office registry for cancellation.
